该中心是一个包括各类影像设备和医学软件的医学影像平台,该平台将为脑数据平台的发展起到重要的推动作用。同时,在脑数据平台的建设中,广泛布局的医疗设备如影像仪器,以及将人性化的扩展体检中心、家庭和个人手机客户端引入商业模式,纳入相关医保支付范畴,打造具有核心自主知识产权的“互联网+智能医疗健康”产品。

研究进展


林庆波教授团队通过多方合作,包含台湾乐生疗养院精神部苏宗伟主任、台中荣总精神部蓝祚鸿主任及其团队、以及英国剑桥大学Edward Bullmore教授,结合临床、影像以及计算模型等领域,揭开隐藏在精神分裂症之大脑功能网络系统的内在表现型(Endophenotype)。 精神分裂症是一种精神疾病,主要在情绪、思考、与感觉产生混乱,常见的征状包含幻听、幻觉、以及妄想等。精神分裂症具有高度遗传性,过去研究已证实患者大脑有不正常神经连接现象。近年来,功能性磁共振造影技术借由非侵入的方式侦测脑区之间的运作模式,可将大脑描绘成由点与线所构成之网络系统。本研究运用静息态功能性磁振影像,发现黄种人精神分裂症患者的大脑网络连结失衡,呈现较为随机化网络架构,与过去白种人为主体之研究结果相吻合,说明大脑网络随机化是精神分裂症中跨人种的重要标记。同时也发现,精神分裂症患者的大脑网络在主要神经网络节点受到攻击(例如大脑受其他疾病影响导致脑区无法运作)时,有较高的整体网络抵抗力,当精神分裂症患者的大脑网络受到攻击时,则依旧维持在过高的连结效率。一般健康人大脑均处于最佳化网络连结状态,但当主要脑区受到攻击时,整体连结效率会衰退得非常快(如同交通网络,当主要城市交通瘫痪时,则无法完成整体工作)。更重要的是,本研究团队发现在精神分裂症患者的健康近亲(包括父母、子女或兄弟姐妹)的大脑网络中,也有相似的随机化连结情形,都较一般健康受试者的神经网络杂乱;同时,患者与其近亲的大脑对于脑网络架构受到其他攻击时,皆有较高的抵抗性,显示大脑网络随机化及对于网络架构受攻击的抵抗力可能为精神分裂症之内在表现型或者遗传风险标记。

研究团队发现不仅精神分裂症患者有随机化大脑网络架构,他们的健康近亲的大脑网络亦有相似的随机化脑网络架构,称为潜在的内在表现型,也可能是未来早期诊断或预测高危险族群转换的依据。然而,此大脑网络内在表现型可能对于其他神经精神相关疾病的攻击具有较强的抵抗力,也可能说明了基因型对于精神分裂症之生物选择性及持续性。此次研究发现精神分裂症的大脑受基因调控所构成之神经网络特性,解开隐藏在病症下的内在表现型, 是此病症的专一性,也可能是抵抗其他病变的优势所在,无论在神经科学领域以及临床医学上,皆具有相当大的贡献与意义。

对外合作


我们与华山医院精神科以及上海精神卫生中心已建立了长期且深入的合作关系,在脑影像采集的项目已启动多年,特别是上海精神卫生中心对于老龄化问题开展了长期的研究,积累了大量的研究资料,也在积极筹划采集影像、基因、行为等多尺度老龄化数据。本研究院胡小平教授在脑成像机制的研究成果是目前脑成像领域的生理基础,也为复旦今后开发新的脑成像方法做好了充分的准备。他同时也是高场磁共振成像的先驱者,在高场上的工作为目前高场磁共振成像,尤其是脑成像,已为脑科学研究奠定了基础。此外,胡教授的小组在分子影像的成果,包括开发了磁共振成像的报告基因与纳米磁颗粒的定量成像,以及波谱成像在脑肿瘤药物治疗的评估和跟踪。在弥散成像方面,本研究院林庆波教授在复杂弥散磁振影像的验证和临床医学应用皆已有许多卓越的研究发表,做出独特的成绩。
